Moving into an aged care home requires paperwork to be completed. In this section we will outline the documents and forms you will need to provide and complete to make your application to an aged care home as simple as possible.
In order to apply for a place at one of our homes you will need to complete the form ‘Opal Aged Care Application Form’. If the form is signed by your Power of Attorney you need to ensure you also attach the relevant authorising documentation.
